As a psychiatric nurse practitioner, you will likely encounter patients who suffer from various mental health disorders. Not surprisingly, ensuring that your patients have the appropriate psychopharmacologic treatments will be essential for their overall health and well-being. The psychopharmacologic treatments you might recommend for patients may have potential impacts on other mental health conditions and, therefore, require additional consideration for positive patient outcomes. For this Assignment, you will review and apply your understanding of psychopharmacologic treatments for patients with multiple mental health disorders. To PrepareReview the Learning Resources for this week.Reflect on the psychopharmacologic treatments that you have covered up to this point that may be available to treat patients with mental health disorders.Consider the potential effects these psychopharmacologic treatments may have on co-existing mental health conditions and/or their potential effects on your patient?s overall health. To complete:Address the following Short Answer prompts for your Assignment. Be sure to include references to the Learning Resources for this week.In 3 or 4 sentences, explain the appropriate drug therapy for a patient who presents with MDD and a history of alcohol abuse. Instructions: Respond by extending, refuting/correcting, or adding additional nuance. Response must be in current APA format, must be grammatically correct and must be at least 200 words.Clinical Manifestations of Acute Kidney InjuryThe possible types of Acute Kidney Injury as per M.r J.R clinical manifestation are perenal injury and intrinsic injury. In perenal injury, the kidney may normally be functioning, but renal perfusion linked to vomiting or diarrhea results in a decreased arterial filtration rate. Some symptoms of perenal injury include nausea, fatigue, and vomiting. And as from the presented case, M.r J.R has shown these symptoms. Besides, there are clinical manifestations of gastroenteritis and possible renal injury. These are possible signs of perenal injury type of Acute Kidney Injury. The intrinsic injury happens when there is injury to the kidney, resulting to loss in kidney function. A common type of intrinsic injury is acute tubular necrosis, with the usual cause such as nephrotoxic (toxic agents to the tubular cells) (Rahman,ÿShad & Smith,ÿ2012). Some of these toxic agents may include medicine. They are more severe when the patient has an existing condition that increases risk of having acute kidney disease. As from the presented case, M.r J.R has been using pain-relieving medicines such Pepto-Bismol, which can be harmful if overused with existing kidney disease. Toxic medicines may limit the normal homeostatic responses to volume depletion, thus resulting in the decline of renal functions. This type of acute kidney has symptoms such as nausea and fatigue, which were presented by M.r J.R. A clinical manifestation of possible renal injury is linked to this type of acute kidney injury.Risk FactorsThere are various risk factors for acute kidney injury. From the given case, Mr. J.R might have the following risk factors.Age: Mr. J.R is 73 years old, and people older than 65 years are susceptible to developing acute kidney injury; this is due to the functional deterioration of the kidney due to the aging process and a reduced renal reverse (American nurse, 2015).Sepsis: Sepsis can be a risk factor in Mr. J.R’s case, mainly because he has infections such as diarrhea and inflammatory response during this process can lead to the adaptive response of the tubular epithelial cells, which reduces functions of the cells to minimize energy demand, and can result in reduced kidney function.Toxic medications can also be a risk factor for kidney disease since they limit the normal homeostatic responses to volume depletion, resulting in the decline of renal functions.Complications On Hematologic System (Coagulopathy and Anemia) and the Pathophysiologic Mechanisms InvolvedIn this case, hematologic system functions will reduce significantly. This is because damaged kidneys generate fewer erythropoietin (EPO), a hormone responsible for indicating bone marrow to create red blood cells. With reduced EPO, the body will make fewer red blood cells signifying less oxygen is supplied to other body tissues. Besides, chronic kidney disease can lead to low levels of nutrients such as vitamin B12 and folate to make red blood cells (Nih.gov, 2014). The case of fewer red blood cells leads to anemia. Besides, chronic kidney disease can lead to venous thromboembolism, making it easier for the body to form blood clots; this is mainly seen with kidney problems causing welling or higher levels of proteins in the urine.Ms. P.C. Case Study QuestionsClinical Manifestations and Microscopic Examination of the Vaginal DischargeThe most possible diagnosis for Ms. P.C is a pelvic inflammatory disease. This disease mainly happens when sexually transmitted bacteria extends from the vagina and affects the uterus, fallopian tube, or ovaries. Ms. P.C has presented clinical manifestations such as abdominal pain, nausea, emesis, and vaginal discharge, which is thick, greenish-yellow in color, and very smelly; these are all symptoms of pelvic inflammatory disease (Higuera,ÿ2019). Besides, the positive high white blood cells count in her microscopic examination of the vaginal discharge is a positive sign for pelvic inflammatory disease.Microorganism InvolvedThe microorganism involved is bacteria. Greenish-yellow in color, smelly vaginal discharge, and abnormal white blood cells, as seen in the case of Ms. P.C, are signs of bacterial infection (Higuera,ÿ2019). This bacteria travels from vagina to womb or fallopian tubes, causing an infection. Several types of bacteria can cause pelvic inflammatory disease; however, the common types are gonorrhea or chlamydia infections. They are mainly acquired during unprotected sex (Higuera,ÿ2019). Besides, someone can get bacteria when they gets into the reproductive region when the usual barrier, which is formed by the cervix, is disrupted; this happens mostly during menstruation (Mayo clinic, 2019). As from the Ms. P.C case, she admits to having unprotected sex once in a while, and her LMP also ended three days ago. These can be one of the causes of the bacteria.Criteria to Recommend HospitalizationTreatment of pelvic inflammatory disease is essential since it helps relieve acute symptoms, exterminate the current infection, or minimize the disease’s long-term effect. This as well helps in the reduction of the transmission of the infection to a sexual partner. A patient can be urgently admitted to the hospital if they have severe symptoms such as the ones shown by Ms. P.C., such as nausea and vomiting. Another factor is when the patient has signs of pelvic peritonitis. The criteria I will use to recommend hospitalization to Ms. P.C include administering antibiotics. The first treatment line is to prescribe antibiotics immediately to Ms. P.C after receiving the lab test. The recommended antibiotics are penicillin, cefoxitin, metronidazole, ceftriaxone, and doxycycline (Das, Ronda & Trent, 2016). They help in stopping the growth of the bacteria or kills it. The patient should then be checked after three days of taking the antibiotics to monitor and ensure the treatment is working. It is also imperative to advise the patient to continue taking the antibiotics to finish the dose even if the condition improves. If the oral antibiotics do not work or in the cases of severe symptoms, the patient will continue in the hospital, where the intravenous medication will be recommended (Das, Ronda & Trent, 2016). Surgery can also be considered when the patient shows signs of scarring on the fallopian tubes. One or both of the fallopian tubes can be removed. Treating Ms. P.C’s sexual partner will also be recommended to avoid the bacteria’s reinfection. While out of the hospital, Ms. P.C will be advised to refrain from any sexual activities until the treatment is completed.ReferencesRahman,ÿM., Shad,ÿF., & Smith,ÿM. (2012). Topic 4 DQ 1An independent variable is one that stands alone. It will most likely influence the dependent variable. A dependent variable is what happens because of the independent variable. Extraneous variables are anything that could affect the research study (Flannelly 2014). For example, if the research topic was whether high concentrations of vehicle exhaust impact incidence of asthma in children, vehicle exhaust is the independent variable while asthma is the dependent variable (NIH 2017). The extraneous variable would then be any other different exposure that could also cause respiratory issues such as secondhand cigarette smoke or living or playing near factories (NIH 2017). One way the researcher could control an extraneous variable is to also put it into the research (NIH 2017). The sample they take of people need to all be exposed to the same factory or cigarette smoke (NIH 2017). Another way they could control the extraneous variable is to take it out all together and choose a sample from somewhere that has high incidence of car exhaust but no factories and none of the individuals participating exposed to secondhand smoke (NIH 2017).using 200-300 words APA format with at least two references to support this discussionCompare independent variables, dependent variables, and extraneous variables. Describe two ways that researchers attempt to control extraneous variables. Support your answer with peer-reviewed articles.

Module 02 ContentInterpersonal Process Recording (IPR)PurposeThis assignment will help prepare you for conversations with clients with mental health conditions.Course CompetencyDescribe the foundations of mental health nursing.Transferable SkillCommunication: Displaying capability in writing, reading, and oral communication; understanding of non-verbal language.InstructionsIt will be carried out with two students working together. This is a ?role-play exercise,? One student is the student nurse, and the other will role-play as a client. In this assignment, the nursing student will demonstrate the therapeutic nurse-client relationship and analyze the therapeutic and nontherapeutic techniques used. You will take turns in the roles. This is not about always having therapeutic responses. It is about learning from practice and review. It is expected that you will think of other ways to respond or interact with the client that may be more therapeutic after the interaction is terminated. After the conversation is over, you will complete in IPR form. The client roles are:Mr. Jones is a 69-year-old retired engineer. He was admitted to the in-patient psychiatric unit the previous day. His daughter had called the police when he locked himself in his bathroom and refused to come out. She thought he was suicidal. He was brought to the hospital by the police and was admitted on involuntary status. Since admission, he has been reticent but told his daughter he had no reason to live since his wife died.Diagnosis:ÿMajor Depressive DisorderNursing Diagnosis:ÿRisk for suicide.Mrs. Alvarez is a 34-year-old female who has lived in this country for the past 10 yrs. She is a stay-at-home mom with three small children. Her husband works two jobs to support the family. Lately, she has been extremely anxious and fears that her children will become ill or injured. This seems to be an unrealistic concern, but she has been unable to sleep well and has lost 15 lbs. in the past month. She is a voluntary admission and states she knows she needs help.Diagnosis:ÿGeneralized Anxiety DisorderNursing Diagnosis:ÿIneffective copingYou will select the role you wish as a client and take turns switching roles. After the conversation, you will document the interactions using the form provided and following the guidelines instructions for each column.IPR Form ExplanationThe IPR is completed with the form to guide you and the rubric to explain the grading process for content.IPR FormTheÿfirst columnÿis for you, the nursing students, comments/ questions, and responses. You may want to include the introduction process. It is understandable if the initial conversation is more superficial to establish rapport. You may not want to include all of this but move on to the more focused interaction. Your nonverbal communication is included here. Then, the conversation will focus on the client?s feelings, concerns, issues as they have identified them. After the introduction, it may be beneficial to establish a short-term goal for the interaction with the client.Theÿsecond columnÿis for the client?s comments, questions, responses. Also, the nonverbal reactions of the client are included in this column.Theÿmiddle columnÿis for you to write what you are thinking or feeling.Theÿlast two columnsÿare for you to document the technique that you used in that portion of the conversation. If it is a non – therapeutic response, you must write the response and think of how to revise it to more therapeutic response. You can change any response if you think of how you could have responded differently but, identify and change all non – therapeutic responses.

Discussion: Diagnosing and Managing Gynecologic ConditionsGynecologic conditions can be difficult to diagnose for a variety of reasons, including overlapping symptoms, lack of patient knowledge, or even patient fear or embarrassment about sharing information. Your role provides you the opportunity to develop a relationship of trust and understanding with these patients so that you can gather the appropriate details related to medical history and current symptoms. When caring for this patient population, it is important to make these women an integral part of the process and work collaboratively with them to diagnose and develop treatment and management plans that will meet their individual needs. For this Discussion, consider diagnosis, treatment, and management strategies for the patients in the following four case studies:Case Study 1:A 32-year-old African American female is concerned about increasing dysmenorrhea over the past three years. In the past year, this was associated with painful intercourse. She has been in a monogamous relationship with one male partner for the past five years. They tried to have children without success. Menarche was at age 10; menstrual cycles are 21 days apart and last for 6?7 days. The first day of her last menstrual period was 10 days ago and was normal. She denies vaginal itching or discharge. On gynecologic exam there was no swelling, external lesions, or erythema, urethral swelling, or vaginal discharge. Cervix is pink without lesions or discharge. Uterus was small, retroverted, and non-tender. Adnexa were small and non-tender. Nodules are noted along the cul de sac.Case Study 2:A 42-year-old African American female is in the clinic for a routine gynecologic exam. When asked, she admits to noticing bleeding in between her menstrual periods for the past several months. She has been pregnant three times and has three children. She is sexually active with one male sex partner in a monogamous relationship. During her bimanual exam, you note an irregular intrauterine non-tender mass about 4 cm in diameter. The mass is palpable abdominally. The remainder of her gynecologic exam was normal.Case Study 3:A 48-year-old Caucasian female is in the clinic concerned about prolonged menstrual bleeding for three weeks now. Her prior menstrual periods have been irregular for the past eight months, lasting no more than three days each. There have been one to two months when she had no menstrual cycles at all. She reports occasional hot flushes and mood swings.Case Study 4:A 16-year-old Caucasian female comes to the clinic concerned because she has not had a menstrual period for three months. She?s a junior in high school and active in sports. She has lost about 10 lbs. in the past two months. She is currently 5 ft. 4 in. and weighs 100 lbs.To prepare:Review Chapter 26 of the Schuiling and Likis text and Chapter 7 of the Tharpe et al. text.Review and select one of the four provided case studies. Analyze the patient information.Consider a differential diagnosis for the patient in the case study you selected. Think about the most likely diagnosis for the patient.Think about a treatment and management plan for the patient. Be sure to consider appropriate dosages for any recommended pharmacologic and/or nonpharmacologic treatments.Consider strategies for educating patients on the treatment and management of the sexually transmitted infection you identified as your primary diagnosis.By Day 3Post an explanation of the differential diagnosis for the patient in the case study you selected. Provide a minimum of three possible diagnoses and list them from highest priority to lowest priority. Explain which is the most likely diagnosis for the patient and why. Then, explain a treatment and management plan for the patient, including appropriate dosages for any recommended treatments. Finally, explain strategies for educating patients on the disorder.
